I think option C could also be a reason. If the Uses Per Customer on the Cart Rule is set to 1 and the Customer has already used the Coupon Code, it would be invalid.

I agree with Linwood. Another reason could be that the Customer is assigned to a Customer Group that is excluded in the Cart Price Rule.
