It could also be the filters referencing restricted Value Lists. That's a common problem.

I'd go with Option D. Restricted Value Lists could definitely cause the report to be inaccessible to some users.

Option B seems the most likely. If the report definition is missing the Execute permission, it would prevent others from accessing it.
