Options C and D make the most sense to me. If the service hasn't received any traffic for a while, or if the request rate has been incremented a bunch of times, it's probably time to move on from the slow start phase.

Option C seems to be the most logical choice here. If the service doesn't receive any traffic for four consecutive increment intervals, it should be taken out of the slow start phase.

I believe another circumstance is when the request rate has been incremented 200 times. That's when automated slow start ends.
