Shouldn't the room be secured even when there's no PHI stored there? This is a disaster waiting to happen. I'm going with D) Authorization and/or Supervision as the best option.

Ah, the joys of healthcare security! I bet the maintenance guy was like, 'PHI? What's that?' D) Authorization and/or Supervision is the way to go, no doubt about it.

Wow, that's a tough one. I guess the lack of locks on the file cabinets is a major security flaw. E) Integrity Controls could be a good answer, but I'm leaning towards D) Authorization and/or Supervision.

Hmm, I'm not sure about this one. Maybe the maintenance personnel should have been supervised more closely? C) Accountability seems like a good option too.

But what about C) Accountability? Shouldn't there be measures in place to ensure those who access PHI are held accountable?
