I'm betting on A, B, and D. You can't just expect the browser to magically find the Portal URL without the proper DNS setup. And the web server and URL template better be on point too.

Aha! Looks like the DNS server IP address is the culprit. Without that, the browser won't know where to go. And C is a no-brainer - HTTPS redirection has to be enabled for the whole thing to work smoothly.
