I believe HBA card failure could also be a potential reason for the LUN loss.

Definitely C) switch port exceptions and D) HBA card failure. Those are classic points of failure in a SAN setup. But I'm not sure about A) RAID LUN resource invalid - that one seems a bit obscure.

I think the main reasons are C) switch port exceptions, D) HBA card failure, and A) RAID LUN resource invalid. The FC SAN environment is pretty complex, so these are the most likely culprits for a LUN loss issue.
