I agree, CSV is the way to go. It's simple, clean, and compatible with just about every data processing tool out there. PDF and DOCX wouldn't really make sense for this kind of data export.

Definitely go with CSV. It's the most versatile and widely accepted format for exporting survey data. Excel and other data analysis tools can easily work with CSV files.
