Forms? Nah, I'd use Planner. That way, you can create a whole project plan around your survey. Who needs analytics when you've got Gantt charts?

I'd go with Forms too. It's a Microsoft 365 staple, and it checks all the boxes for this survey requirement. Easy-to-use and powerful.
